Am I eligible for GAFC?
To enroll in Group Adult Foster Care, you must:
​
  • Be at least 22 years of age
  • Be eligible for MassHealth Standard
  • Live in an assisted living facility or other housing that participates in GAFC
  • Require daily assistance with at least one Activity of Daily Living (ADL), which is related to a medical diagnosis
  •  Have had a physical by your primary care physician (PCP) within the past 12 months
